What happens when the exam is complete?

The paramedical questionnaire and any additional requirements are forwarded to the insurance company. Any specimens obtained during the examination are sent to LabOne or another designated laboratory and the results fowarded to the insurance company for assessment.  Each insurance company has it’s own policy on sending the results of the lab tests to you.   Check with your agent to find out more about this.  Absolutely no results are sent to ExamOne 3072.
 

Applicant Instructions:

In order to obtain accurate information, it is recommended the applicant:

  • Fast for 3-4 hours prior to the appointment up to 12 hours
  • Limit coffee and smoking one hour prior to the appointment
  • Drink a glass of water one hour prior to the appointment
  • Refrain from drinking alcoholic beverages for at least twelve hours prior to the appointment
  • Provide names and dosages of current medications
  • Provide any history of problems associated with providing a blood sample
  • Have available member number/s of any current health insurance plan
  • Have available names, addresses and phone numbers of any doctors or clinics visited in the last 5 years
  • If your appointment needs to be rescheduled, call the ExamOne 3072 office up to one hour prior to reschedule.  The number to call is (818) 557- 6300
